The most useful records are the kind the pages already cite: a provider's own web page, its Google listing, or another public document such as a published board decision. A link is best. If the document is not online, a short description of where to find it is enough.
Use the same form when a page no longer matches its source, for example when a provider's website has changed what it says about who performs treatment.
